PUBLIC HEALTH PRACTICES IN THE WORLD AND IN OUR COUNTRY
An international conference on Primary Health Care (Primary Health Care: IBSH) was organized in Alma-Ata, the capital of the Kazakh Soviet Socialist Republic, between September 6-12, 1978. 30 Years After the Alma-Ata Conference, were the principles in the Declaration of Primary Health Care implemented? Were the goals aimed at with Primary Health Care (PHC) achieved? Was the PHC approach able to surpass traditional health services? Unfortunately, it is not possible to answer yes to all of these questions with peace of mind. The answers to these questions are included in the World Health Report published by the World Health Organization (WHO) in 2008, 30 years after the Alma-Ata Declaration. According to this report, the “health for all” approach could not guide the health systems of countries as much as expected. One of the reasons for this is that the PHC approach, which was prepared by taking into account the conditions of 1978, could not meet the developing and changing expectations. In addition, the issues of “society participation in health services” and “intersectoral coordination”, which are emphasized in the declaration, are also difficult to implement and realize. For this reason, traditional health services before the TSH Declaration continued in some countries. According to the 2008 World Health Report, there are five basic common problems in health services in countries as of 2008: 1-Reverse Service, 2-Impoverishing Service, 3-Fragmented Service, 4-Unsafe Service, 5-Misdirected Service. The political, social, economic, technological and scientific changes that have occurred in the 30 years since the TSH Conference have caused the importance of many health problems to decrease, while new problems have emerged. People now live longer than 30 years ago, and babies die less. The resources allocated for health are increasing. There have been major changes in the society’s understanding of health. People can now access information on health issues more easily. On the other hand, health services have been inadequate in meeting changing expectations and coping with new problems. In light of this information, the purpose of this report is to evaluate public health practices in the world and in our country.
